The C&MA Benefit Office is excited to welcome the Rocky Mountain District into the C&MA Health Plan. The Rocky Mt. District becomes the 15th district to participate in our US Health Plan.

After further communication with Highmark, it was discovered that the change from Labcorp to Quest Diagnostics IS NOT a nationwide change. Please go to the Highmark BCBS website (https://www.highmarkbcbs.com) and click on “find providers” to find in-network labs specific to your area. Thank you for your patience and understanding regarding this matter.

For International Health Plan participants, please note CBSA changed its name to Meritain.  
 
New website: www.myMERITAIN.com 
If you already have a log-in, it will still work. If you do not yet have a log-in, I would encourage you to set one up so that you can view your claims online, as this will save you weeks compared to waiting for your EOBs (Explanation of Benefits statements) to arrive in hard copy.

    If you are on our C&MA Health Plan, please enroll your newborn baby or new adopted child within 30 days of birth or adoption. Late enrollment results in a 6-month waiting period before coverage will begin, and leaves parents responsible to pay for well baby visits or any illness throughout those months. 
     
    If you know someone who is expecting a child, please remind them to call the Office of Employee Benefits to request a form. It’s very easy to enroll your child and simply needs a completed enrollment form faxed or mailed to us within 30 days of birth. 
     
    By the way, the same 30-day rule also applies to other life events such as a new marriage, change of employment, or involuntary loss of other coverage. Please always call our office at 800-700-2651 and ask about your specific event. We are always delighted to take time to help you understand what is needed.

    If you are on our C&MA Health Plan for US Employees, the following information applies to you. 
     
    When you need health care outside of the U.S. follow these steps: 
    1. Always carry your Highmark identification card. 
    2. If you need emergency medical care, go to the nearest hospital. Call the BlueCard Worldwide Service Center at 1-800-810-BLUE (2583) or call collect at 1-804-673-1177 if you're admitted. 
    3. If you need non-emergency inpatient medical care, you must call the BlueCard Worldwide Service Center. The Service Center will facilitate hospitalization at a BlueCard Worldwide hospital or make an appointment with a doctor. It is important that you call the BlueCard Worldwide Service Center in order to obtain cash-less access for inpatient care. The Service Center is staffed with multilingual representatives and is available 24 hours a day, seven days a week. 
    4. Call Highmark by using the number on the back of your card for pre-certification or prior authorization, if necessary. When in doubt if necessary or not, call Highmark.  
     
    When you are away from home within the U.S. these steps: 
    1. Always carry your Highmark identification card.  
    2. In an emergency, go directly to the nearest hospital.  
    3. To find names and addresses of nearby doctors and hospitals, visit the http://www.bcbs.com/healthtravel/finder.html or at 1-800-810-BLUE.  
    4. Call Highmark for pre-certification or prior authorization, if necessary (refer to the phone number on your BCBS ID card). When in doubt if necessary or not, call Highmark. 
    5. When you arrive at the participating doctor's office or hospital, simply present your BCBS ID card. 
    If you wish to learn more about your out-of-area coverage, visit the BlueCard information by going to www.bcbs.com, click on the Healthcare Coverage tab (found in the middle/top of the page) and than on the BlueCard link.

    Go to Consumer Toolkit found in the Toolkit box on the lower right-hand corner 
    Type in the Subscribers social security number 
    Type in the Subscribers date of birth (MM/DD/YYYY) 
    After the site opens there is a choice to ‘Print ID Card’ on the left-hand column, simply follow these instructions

    Please note the information in Employee Benefits is intended as a summary of benefits. It does not guarantee coverage. 
     
    The Health Plan pays benefits for medically necessary services resulting from illnesses or injuries that are not work related, up to reasonable and customary limits. Even though a doctor recommends or performs a service for you or your covered dependent, this does not automatically mean the service is covered by the Plan. 
     
    If you have any doubt about whether a proposed treatment or service is covered under the Health Plan, please contact the number shown on your ID card.

    The Fellowship Fund is the benefit pension plan created in the 1930s that still supports about 600 C&MA retirees (including missionaries and pastors). The fund to cover these retirees’ allowances is underfunded, causing General Council to mandate contributions from churches and other C&MA entities until it is fully funded.  
     
    According to the mandate approved by Council “each church affiliated with the C&MA is required to contribute each year an amount equal to .7% of its local church operations income from the prior year. A newly established church is required after the first year to pay 50% of the normal church contribution amount, or .35% for the first five years.” 
     
    Churches may make these contributions by automatic withdrawal each month, through using the FF Bank Authorization Form. Thank you for giving to the Fellowship Fund, fulfilling our debt of honor to retirees who gave their lives in service.
