A Delta Dental member may print a dental card by:
Visiting www.deltadentalco.com
Go to Consumer Toolkit found in the Toolkit box on the lower right-hand corner
Type in the Subscribers social security number
Type in the Subscribers date of birth (MM/DD/YYYY)
After the site opens there is a choice to ‘Print ID Card’ on the left-hand column, simply follow these instructions
